Momenta Newcastle is a leading UK provider of evidence-based health behaviour change programmes. Commissioned by NHS England, we deliver the 'Type 2 Diabetes Path to Remission Programme' across multiple regions.
We're looking for a number of in-person Health & Wellbeing Coaches to work with us on a freelance basis to help deliver our 1:1 NHS T2DR sessions in person, supporting participants through a structured year-long journey toward Type 2 diabetes remission.
We are specifically looking for coaches to cover the following areas:
South Yorkshire - Sheffield / Rotherham
Midlands - Birmingham and Solihull
North East - Newcastle / Darlington
Somerset - Taunton / Yeovil
Leicestershire - Leicester
Cheshire & Merseyside - Chester / Liverpool
What you’ll be doing
You will be supporting the delivery of the NHS Type 2 Diabetes Path to Remission programme through in-person, 1:1 coaching. You'll empower participants to improve their health outcomes over a structured 12-month journey, aiming for remission where possible.
Delivering one-to-one sessions in local health or community venues
Building rapport and tailoring support to participant needs
Supporting long-term behaviour change through structured coaching
Documenting all sessions and tracking outcomes
Attending regular team meetings and ongoing training
